DAVENPORT — Patients’ protected health information was improperly discarded in a dumpster by an employee of BioReference Laboratories, the company said Friday.
The records contain patients’ first and last names, dates of birth, addresses, medical record numbers, Social Security numbers, insurance information, diagnosis codes, and descriptions of the lab tests ordered. Financial information was not in the records.
The company said it has found no evidence that any patient information has been improperly used or accessed.
People who may have been affected should monitor their financial statements and report any suspicious activity, and consider using the credit monitoring services that BRLI is offering at no cost.
For more information, affected people may call the company’s incident hotline at 855-287-9334 from 9 a.m. to 6 p.m. Monday through Friday.
